Initial Comments:
Based on the findings of an offsite followup survey completed December 21, 2022, Lady of Fatima Health Services, had corrected the deficiencies cited under 42 CFR, Part 484, Subparts B and C, Conditions of Participation: Home Health Agencies. The deficiencies were cited as a result of a Medicare recertification and state relicensure survey completed June 27, 2022.
